Welcome to Week 3! Last week, we covered root operations, a key component of ICD-10-PCS coding.
You have probably discovered that you may need to reference a medical dictionary and/or anatomy and
physiology resources throughout the course. That is very common, so keep those resources handy. As a
reminder, the encoder software also has some great resources and reference materials built in, so if you
have not been using the software yet, this might be the time to check it out.

This week, we will take a look at the anatomical regions and body systems for use in coding. Procedures
are coded based on the root operations performed on a specific body part within a specific body system.
By now, you should be seeing that each week builds upon the last. Recall that you must assign a value to
all seven characters in order to build a procedure code. Body system, root operation, and body part
represent Characters 2-4 in the Medical and Surgical section. Let’s dig in!

Terminal Course Objectives

3 Given a sample patient record, analyze procedural statements and operative notes to determine
the objective of the procedure and the applicable root operation.

Key Concepts

Describe the use of the root operation, control, in the anatomical regions’ body systems.
Discuss the other applicable root operations for general regions, and the upper and lower extremities,
including detachment, division, alteration, and creation.
Discuss the 20 applicable root operations for the central nervous system.
Discuss the 16 applicable root operations for the peripheral nervous system.
Discuss the common root operations used in coding procedures performed on the sense organs.
Discuss the 22 applicable root operations for the mouth and throat and respiratory system.

4 Given a copy of the ICD-10-PCS, demonstrate an understanding of the design and structure, and
the application of each to the steps in the coding process for a given coding scenario.

Discuss the use of qualifier values for bypass in the general regions, and detachment in the upper and
lower extremities’ regions.

Identify the meaning of qualifier value B in the central nervous system for cerebral cisterns.

5 Given a sample patient record, demonstrate an understanding of the attributes, characteristics,
and definitions used in ICD-10-PCS.

Utilize the encoder software (through iLab) to complete coding exercises to accurately code inpatient
procedures from each of the following.
Procedures involving anatomical
regions Procedures of the nervous
system Procedures of the sense organs
Procedures of the respiratory system

The practice homework in this course consists of all practice exercises under “Check Your
Understanding” in your textbook corresponding to the chapters covered each week. These are found at
the end of each chapter and include items such as Coding Knowledge Check, Procedure Statement
Coding, and Case Study Coding.

These will not be handed in or graded, but it is strongly recommended that you complete the practice
homework in order to enhance your understanding of the course material and prepare yourself for
other assignments and exams in this course. In addition, be sure to check out the Just For Fun section to
the left for more practice exercises.

Coding Roundtable Group Discussion

You have each been separated into small groups for Coding Roundtable Group Discussions. These are
graded separately from the Discussion boards but have similar requirements in terms of frequency.
You must post on a minimum of 3 separate days per week, and your initial post must be in by
Wednesday. Your instructor will post the answers on Thursday, so late posts are not accepted and do
not earn credit in the coding roundtable. You will find your individual group listed as one of the
sections to the left in each week.

In the Coding Roundtable for this week, you will utilize the Dissecting an Operative Report Worksheet
(Textbook Chapter 2, Figure 2.6) to break down the operative report provided in the case scenario, and
build the appropriate code(s) for the procedure.

Coding Roundtable Student Expectations

Students are expected to post (respond) to the coding roundtable topics on three separate days per
week, beginning no later than Wednesday.

The first post must completely address the roundtable topic or case study for the week (worth
10 points).

Full-credit posts demonstrate: understanding of the topic;
application of principles related to subject matter;
application coding guidelines (case study topics); and
use of course materials (i.e., coding manuals, encoder, coding guidelines, etc.).

Second and third posts must respond to fellow learners (worth 5 points each).

Full-credit posts demonstrate: providing additional information to the discussion;
elaborating on previous comments from others; and
presenting explanations of concepts or methods to help fellow students, using examples where
appropriate.
iLab

You must use the encoder software (instructions available in the iLab area of each week) to assist in
completing your weekly homework assignments and Coding Roundtable Case Studies. This is a resource
used in addition to your coding manual. There are no deliverables actually turned in through the iLab.

As we learned in Chapter 1, the second character in the ICD-10-PCS code represents the body system,
the general anatomical region, or the physiological system involved in the procedure.

ICD-10-PCS includes an anatomical regions body system for use in describing general locations of the
body. Be sure to review the anatomy and components of these areas. As a reminder, Appendix I of
your ICD-10-PCS coding manual contains some helpful anatomy illustrations that may be useful to you
during the coding process.

CODING NOTE: An important point to remember is that per OGCR B2.1a, “The procedure codes in the
general anatomical regions’ body systems should only be used when the procedure is performed on an
anatomical region rather than a specific body part…or on the rare occasion when no information is
available to support assignment of a code to a specific body part.”

The anatomical regions include the following.

1. General regions (value
X) Body cavities
Larger surface areas of the body (such as head, neck, chest wall, abdominal wall, and upper and lower
back)
Three main tubular tracks (GI, GU, and Respiratory tracts)
2. Upper extremity regions (value Y)—body parts above the central dividing line of the diaphragm
