Age: 70 year old Gender: Female
Subjective Data:
Chief Complaint (CC):
“Being sad”
History of Present Illness:
The client has in the recent pas had a hearing loss in both ears. Also, the client was been
identified for cochlear implants at some point in the near future. Conversely, there has been no
surgery scheduled. The client crying frequents for no clear reason and her sadness has increased
in the past one year. Furthermore she reports that she has lost interest in things that’s she enjoyed
doing and had frequents fatigued and losses concentration. She is very clear in her stand that she
has no feelings of sadness. It is evident that her spouses died many years ago and has been living
alone with assistance from the home based workers. The son and the homeworkers do visits
frequently, she still have difficulty staying alone and going about her duties. Apart from the
episode off sadness, the clients claims that’s she has “awful” experience during her sleep. She
even does not fall asleep due to the fear of the for the night and her legs “ache and jumps.” She
does not reports any napping during the day. Having gone to the outpatient supportive
psychotherapy, it appear that her hearing loss makes the projected modalities almost impossible.

CAD
Environmental allergies
Patient Intake:
Gradual onset of geriatric, first-episode MDD symptoms likely as a result
of hearing loss and mobility loss. Lost interest in things she used to enjoy and is fatigued with
poor focus and concentration. Denies feelings of guilt, worthlessness, or any suicidal thoughts.
