1. Continuity of Care: Interventions Promoting Independence
Proper organization of various community groups working together can help the continuity of
care and promote independence for the client; such as: family, civic organizations, employment
bureaus, etc.
Community health nurses are essential for maintaining client health and safety throughout the
transition from the hospital to home setting and furthering client education regarding their self-
care.
Discharge planning should always begin on admission to plan client needs after leaving the
hospital; this should include the client, family, nurse, provider, case manager, and other
important members of the health care team.
2. Crisis Management: Crisis Intervention for Intimate Partner Abuse
The nurse should first assess the client for any immediate injuries and potential for homicide or
suicide.
Strategies to decrease client’s stress and anxiety may include: avoiding false reassurance,
listening, remaining with the client, eye contact, etc.
There are four main phases of a crisis, it is important for the nurse to be able to distinguish
between them and know which nursing interventions to apply respectively.
3. Care of Special Populations: Recommending Appropriate Referrals
For referrals to be given to the correct agencies/services the community health nurse should
fully assess the client’s unique situation and determine their needs.
Services available in most cities include: housing, providers, therapy, support groups,
employment assistance, alcoholics anonymous, narcotics anonymous, educational programs,
WIC, MADD, etc.
When gathering information from the client make sure to use a nonjudgmental, matter-of-fact
tone to establish rapport and ensure the client is comfortable talking about their situation.
4. Client Education and Discharge Teaching: Relieving Brest Engorgement
The nurse should teach the client to apply cold compresses to the breast following feedings to
assist with breast engorgement.
Taking a warm shower prior to feedings can stimulate the letdown reflex and milk flow.
Educate the client on completely emptying her breast during feedings and allow the infant to
feed on demand.
5. Prenatal Care: Managing Heartburn in the Second Trimester
Onset of heartburn is usually due to displacement of the stomach by the growing uterus and
slowing of gastric motility and digestion caused by increased progesterone levels.
Educate the client on small frequent meals that don’t make them feel too empty or too full and
to remain upright for at least 30 minutes following meals.
Teach the client not to eat late at night which can cause heartburn.
6. Assessment and Management of Newborn Complications: Neonatal Effects of Maternal
Cocaine Use
When caring for an infant who is experiencing withdrawal from cocaine, it’s important to avoid
eye contact, vertical rocking, and giving the infant a pacifier.
