The eighth edition of Nursing in Today’s World: Trends, Issues, and Management, like the previous editions of this textbook, focuses on the nonclincal aspects of the professional nursing role. Much of this content is just as critical to practicing safely as is competence in the performance of clinical skills. Nursing continues to experience significant changes as it responds to the transformation occurring in the health care delivery system. As those adjustments and modifications have taken place, we have endeavored to keep the content of this textbook in tune with the times, revising and updating each chapter with the latest information and related Internet resources. Additonally, we have made every effort to be responsive to suggestions provided by our readers. Thus, the eighth edition includes two new chapters devoted to the topics that surround the leadership and management role of the nurse. And we have retained in a concise and readable format the content that is basic to a textbook that has the nursing profession as its focus. Here is what you will find. Text Organization From the onset, we have attempted to design a textbook that can provide a flexible approach to instruction with the intention that each chapter be able to stand alone with references occurring throughout that will lead the reader to related content appearing in other areas of the book. This goal has been maintained so that content can be reordered if so desired. For example, those of you who prefer a chronologic approach to the development of nursing as a profession, are encouraged to adapt your use of the various chapters to match this preference. If you prefer to study the origins of nursing first, that section of this text (Chapter 4) can be read and deliberated before studying the first three chapters. Similarly, some content areas might be combined or perhaps eliminated if it appears elsewhere in the curriculum of your program. The textbook is organized to four major units of study. • Unit I: Economic and Political Aspects of Health Care Delivery We believe that if students are to gain an appreciation of nursing’s place in the health care delivery system, it is best facilitated by gaining an understanding of how the health care delivery system functions and what forces impact its operation. Thus the text begins with an overview of economic and political aspects of health care delivery, including such information as how it got to where it is today, how it operates, how it is financed, and how it is viii Preface politically influenced. Of particular application to nursing is Chapter 3, which is devoted to the relevance of the politial process to nurses, including information describing how one can become informed. It also outlines a variety of ways in which nurses can affect the political process. This chapter also includes a discussion of the many nursing organizations, why so many exist, and the major focus of each. • Unit II: Appreciating the Development of Nursing as a Profession Chapters 4, 5, and 6 focus on nursing itself. By understanding the early origins of nursing and it’s challenges as a profession, and by exploring the various educational avenues providing entry into the profession of nursing, students will be better prepared to participate in the various nonclinical activities and discussions that are part of nursing. Students will learn about the development of the profession and the challenges to that professional standing. The various educational paths that prepare graduates for registered nurse licensure are presented. Issues discussed include the controversy that exists regarding the form of educational preparation deemed most appropriate for entry-intopractice, the implementation of differentiated practice, and whether continuing education should be mandatory or voluntary. • Unit III: Legal and Ethical Responsibility and Accountability for Practice This area of study comprises four chapters. It begins with a focus on legal and ethical accountability and includes a discussion of the credentials found among health care providers, tracing the history of credentialing in the United States and leading to an exploration of the legal issues facing today’s nurse. Also incorporated into this unit is a chapter devoted to ethics that provides a foundation for ethical decision making, including a broad discussion of various bioethical issues that impact nursing and nursing practice. The bioethical chapter will challenge students as they consider the many facets related to stem cell research, gene therapy, and the human genome project. • Unit IV: Career Opportunities and Professional Growth This section of Nursing in Today’s World focuses on career opportunities and professional growth and includes two new chapters devoted to the leadership and management role of the professional nurse. It begins by exploring the various employment opportunities available to nurses today and provides valuable information related to seeking, applying for, and resigning from a nursing position. This is followed by a discussion of the workplace and includes a discussion of organizations, their purpose and function, as well as the topic of collective bargaining as it applies to nursing. The final two chapters discuss the leadership/management role of the nurse. Following a discussion of the various forms of leadership, practical suggestions for team building, motivating others, dealing with conflict, and managing change provide the student with critical information needed to effectively perform in a leadership role.
