Directions: Develop an educational series proposal for your community using one of the following four topics: 1. Bioterrorism/Disaster 2. Environmental Issues 3. Primary Prevention/Health Promotion 4. Secondary Prevention/Screenings for a Vulnerable Population Planning Before Teaching: Name and Credentials of Teacher: NAME RN Estimated Time Teaching Will Last: 40 minutes Location of Teaching: Community Church Supplies, Material, Equipment Needed: Brochures and refreshments Estimated Cost: $20.00 Community and Target Aggregate: Adults (25-35 years old) Topic: Diabetes Type 2-Risk and Prevention Identification of Focus for Community Teaching (Topic Selection): Education on risks of diabetes type 2. Epidemiological Rationale for Topic (Statistics Related to Topic): The number of people with diabetes in United States is growing every year. There were about 34 million diagnosis in 2018, and about 1.5 million new diabetes diagnosis occur every year. The statistical data shows that in addition of 26.8 million people diagnosed with this disease there This study source was downloaded by from CourseH on :36:13 GMT -05:00 were additional 7.3 million people that did not know they had diabetes because they were never diagnosed. The rate of diabetes of 14.3 million is especially high in the group of older Americans, age of 65 and older. According to statistical data, nearly 88 million adult Americans age 18 and older are at prediabetic stage. Financially, the diabetes is a huge burden on United States medical care. It amounted in 2018 to 237 billion dollars in medical cost and another 90 billion dollars in lost productivity (Statistics About Diabetes, n.d). According to American Diabetes Association the key to control the diabetes is to maintain a healthy diet, correct weight, and physical activity. A person is able to lower the diabetes risk with just half an hour of physical activity daily. For best results it is the American Diabetes Association recommendation to start the exercise with just a few minutes and then gradually increase to reach a 30 minutes daily level. This routine will work even for people that were physically not active (Physical Activity, n.d.). Teaching Plan Criteria Your teaching plan will be graded based on its effectiveness and relevance to the population selected. This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion Nursing Diagnosis: Abnormal intake of calories as evidenced by abnormal body weight. Readiness for Learning: Identify the factors that would indicate the readiness to learn for the target aggregate. Include emotional and experiential readiness to learn. The participants will articulate their hesitations or their misunderstandings about how to prevent obesity and diabetes. They will ask questions about how to decrease their risk factors by changing their diet and increase their daily exercises. The open conversation on the topic of diabetes will follow. Learning Theory to Be Utilized: Explain how the theory will be applied. From my experience I realized that the most useful method of teaching is a method which is called a Teach Back method. After I provide all information regarding diabetes prevention, proper nutrition, and weight control I will ask the audience to state in their own words what they learned, and what they should do to improve their health. That way I will make sure that I explained concepts of diabetes and weight control in a manner for the listeners easy to understand (Use the Teach-Back Method: Tool #5, 2015). Using the Teach Back method I will distribute brochures that contain information written in easy to understand language. I will be asking participants open-ended questions for them to answer in their own way. That way I will assess of how much they learned and their understanding of all information that I provided for them. Goal: Healthy People 2020 (HP2020) objective(s) utilized as the goal for the teaching. Include the appropriate objective number and rationale for using the selected HP2020 objective (use at least one objective from one of the 24 focus areas). If an HP2020 objective does not support your 2 This study source was downloaded by from CourseH on :36:13 GMT -05:00 teaching, explain how your teaching applies to one of the two overarching HP2020 goals. D-16 Increase prevention behaviors in persons at high risk for diabetes with prediabetes How Does This HP2020 Objective Relate to Alma Ata’s Health for All Global Initiatives The health inequalities in one part of the world can be affecting other parts of the world according the Declaration of Alma-Ata that was adopted at the International Conference on Primary Health Care in 1978, Therefore, it is important to recognize them as interconnected issues and eliminate them wherever they occur. The diabetes becomes a global health problem. HP2020 D-16 makes people who are at the prediabetic stage, aware that they are at the danger of having a diabetes illness. It educates patients that already have diabetes on how to improve their life. It also educates public to increase awareness of diabetes as a growing problem. Develop Behavioral Objectives (Including Domains), Content, and Strategies/Methods: Behavioral Objective and Domain Example – Third-grade students will name one healthy food choice in each of the five food groups by the end of the presentation. (Cognitive Domain) Content (be specific) Example – The Food Pyramid has five food groups which are…. Healthy foods from each group are…. Unhealthy foods containing a lot of sugar or fat are…. Strategies/Methods (label and describe) Example – Interactive poster presentation of the Food Pyramid. After an explanation of the poster and each food category, allow students to place pictures of foods on the correct spot on the pyramid. Also, have the class analyze what a child had for lunch by putting names of foods on the poster and discussing what food group still needs to be eaten throughout day. 1. The control of diabetes depends on appropriate diet and correct weight. Food choices are part of that control. Participants will try to name these food choices. 1. Fruits, Vegetables, lean meats, eliminating sugary foods are examples of healthy food choices. 1. In the pamphlet given to the audience I will describe the influence of diabetes on the patient’s body. Snacks given to participants before the presentation will include healthy choice such as fruits, vegetables, and unhealthy choice such as cookies. They will be given free choice but will have to 3 This study source was downloaded by from CourseH on :36:13 GMT -05:00 explain the reason of their choice.
