The percentage of Americans age 65 and older remains high at 25.2% or 12.0 million seniors (diagnosed and undiagnosed). 1.5 million Americans are diagnosed with diabetes every year. In 2015, 84.1 million American age 18 and older had prediabetes. Diabetes remains the 7th leading cause of death in the United States with 79,535 death certificates listing it as the underlying cause of death, and a total of 252,806 death certificates listing diabetes as an underlying or contributing cause of death. An individual who is physically active can decrease their risks with 30 minutes of exercise and for those that haven’t been very active recently, the American Diabetes Association recommends starting out with 5-10 minutes a day and then work up to more time each week to get to 30 minutes a day (Physical Activity, n.d.). Nursing Diagnosis: Imbalanced nutrition: more than body requirement related to food intake that exceeds body needs as evidenced by weight gain. Readiness for Learning: Identify the factors that would indicate the readiness to learn for the target aggregate. Include emotional and experiential readiness to learn. The young adults will participate and verbalize their concerns or misconceptions about how to prevent diabetes and obesity will ask appropriate questions about decreasing their risk factors, food choices, and engage in conversation with open body language. Learning Theory to Be Utilized: Explain how the theory will be applied. I will be using the teach back method and theory because this is a useful way of checking our audiences understanding by asking them to state in their own words what they need to know or do about their health and is way to confirm that I have explained things in a manner that the audience has understood (Use the Teach-Back Method: Tool #5, 2015) Through the teach back theory, I will be handing out a pamphlet that uses simple language and will be evaluating their understanding of the material by asking open ended questions having the audience reiterate in their own words what they have learned. Goal: Healthy People 2020 (HP2020) objective(s) utilized as the goal for the teaching. Include the appropriate objective number and rationale for using the selected HP2020 objective (use at least one objective from one of the 24 focus areas). If an HP2020 objective does not support your teaching, explain how your teaching applies to one of the two overarching HP2020 goals. D-16 Increase prevention behaviors in persons at high risk for diabetes with prediabetes.
