Skin lesions and rashes assessment relies on a comprehensive history and physical
examination (Sehgal, 2017). The practitioner has to understand the characteristics of different
skin lesions, clinical appearance, arrangement and distribution of the disorder, and related
pathological conditions (Sehgal, 2017). It is also essential to be familiar with everyday
manifestations of specific lesions such as itching or fever (Sehgal, 2017). Identification of life-
threatening skin conditions and highly contagious conditions should be done quickly and treated
as such (Cash & Bruggemann, 2020).

SUBJECTIVE DATA:
Chief Complaint (CC): Itching, burning, sore, scaly skin
History of Present Illness (HPI): Ken Osborne is a 55-year-old Caucasian male
presenting with itching, burning, sore, scaly rash on his fingernails, palms, knees, elbows, and
back. The onset is reported as gradual for the past month. He reports he had a few lesions around
his fingernails in the past. Ken was diagnosed with psoriasis at age 25 by his PCP. He has
managed the skin lesions with hydrocortisone 0.1% cream and Calcipotriene cream in the past.
He reported that he was exposed to the sun and was in contact with irritants at his brother's farm.
He reports discomfort rated at 5/10 due to itchiness, soreness, and the burning sensation he feels
after taking showers. He denies any fever, new medications, or any lifestyle changes. He denies
