For health care providers to deliver holistic care, the must take into consideration the
physical, mental and spiritual well-being of their patients. Despite the growing awareness of the
importance of religion and spirituality in patient care, it can still be overlooked by health care
providers since it isn’t their main priority in different situations. The Joint Commission has

accreditation so health care providers can make an effort to seek the spiritual needs and insights
of their patients when it comes to their problems and concerns. They have created a guideline for

creating a spiritual assessment tool for patients and after reviewing this, a series of questions was
made in order to conduct an interview regarding the spiritual needs of the interviewee. An
analysis of the interview will also be provided regarding the experience and a deep insight.
Part I: Interview
Transcript
The following transcript provides the interview assessing a subject’s spiritual needs with
the minimum of five questions based on the guidelines of the Joint Commission. The assessment
and interview focused on my father’s experience as a patient with his spiritual beliefs. He is
someone I have looked up to and has been diagnosed with ulcerative colitis.
