Adult Health lll Exam 2
1. Patient with heart failure is lying flat in bed, during an assessment you see they have
Jugular Vein Distention, what should you do? Sit them up about 45 degrees
2. Patient has new prescription of Nitro Tablets – What would you do in their
discharge teaching?
Might get headache, dizziness, so if we are worried if they get dizziness – immediately after you
take them you should lay down so that you won’t fall after taking them.
3. You are getting shift reports and there are 4 patients – Who are we going to go assess
first? Angina reporting chest pain – developing into an MI- see this patient first
4. MI 10 days ago and they are in cardiac rehab and we are teaching of self-monitoring and
when they should stop exercising – what should we include in the teaching- when should
they stop exercising?
Feelings of palpitations or dyspnea
5. Planning a conference for older adults about age related to blood pressure, what would
you include?
Structural and functional – changes in the cardiovascular system that occur w/age contribute
to the increase in blood pressure
6. Your client is unresponsive and has absent peripheral pulses monitor shows
Check a pulse and start CPR
7. Assessing a rhythm and this patient has HF and they are being monitored on remote Tele –
it says ventricular fibrillation (V-FIB) going on …. what labs will we communicate to HCP
0.7 SERUM MAGNESIUM
8. We are caring for someone and they had a MI 12 hrs. ago and their remote tele has absent
P waves and rhythm is irregular and HR > than 100
STROKE – because they are in A-FIB (anticoagulant is needed because AFIB causes strokes)
9. We are caring for someone and suddenly they developed a wide QRS complex, B/P 82/40,
RR 24, not arousable and we can’t feel or palpate a pulse. What action are you going to
take? Defibrillate them
10. Someone has an inferior wall & MI 4 days ago, now they are feeling dizzy with chest
discomfort and B/P of 98/48 and monitor shows a second-degree type 2 block and HR 52-
we should prepare them for what intervention?
Transcutaneous pacemaker

11. Nurse in ER caring for client REPORTING W/ chest pressure & NURSE prepares a 12 LEAD
EKG and notes ST elevations on leads L1, V5, V6.....What kind of heart attack are they
having?
Lateral Wall Myocardial Infarction
12. We are taking care of a patient on a cardiac monitor B/P 97/35 and on the monitor Is
high waves (v-tac) what do we do next:
Assess their pulse
13. How about we are at a local health clinic cardiac clinic and there is a young 25-35-man pre-
employment physical exam and he denies any problems we do a 12 lead and normal PQRST
but the HR is 40 what do we do?
Continue our physical assessment and document our findings / Ask if they are a runner cause
if they do a lot of physical things all the time, their HR at 40 is normal
14. We are caring for a group of patients assigned to us, the nurse should anticipate needing
cardioversion w/ which of the following problems:
Atrial Flutter
15. We are caring for someone with dx of AFIB and scheduled for cardioversion 24 hrs.
from now, what should they have with it? (SELECT ALL THAT APPLY)
*TEE (Transesophageal Electrocardiogram) in them for wall motion and looking for clots
*Have anticoagulants for 4 weeks after the procedure
*Include diet modifications & teaching
16. Caring for someone who develops tachycardic arrhythmia which noninvasive
intervention would we apply: (SELECT ALL THAT APPLY)
*Instruct them to bear down
*Put their face in ice water
*Instruct the client to gag
*Instruct them to hold their breath
17. PROVIDING CARE for a patient with an Implanted pacemaker, what intervention should
we anticipate being prescribed for them?
Telemetry monitoring
18. If we are evaluating someone who is prescribed Atropine IV for symptomatic Type 1
second degree AV block, which would indicate meds were effective?
Increase in the clients HR
19. We receive change of shift report. What patient would want to go see first?
ICD fired twice 30 minutes ago and has a dose of Amiodarone THAT is due
