Ch 1 - Nurse's Role:
Nurses Role- According to the American Nurses Association (ANA); “Nursing is
the protection, promotion, and optimization of health and abilities,
prevention of illness and injury, alleviation of suffering through the diagnosis
and treatment of human response, and advocacy in the care of individuals,
families, communities, and populations.”
Purpose of health assessment- is to obtain subjective and objective data.
Health assessments are usually structured screening and assessment tools
used in primary care practices to help the health care team and patient
develop a plan of care. Health assessment information can also help the health
care team understand the needs of its overall population of patients.
Components of health assessment include conducting a health history,
performing a physical examination, and communicating and documenting the
findings according to agency policy.

• Emergency Assessment- Urgent, life threating or unstable
•
• Comprehensive assessment- Full head-to-toe, physical assessment
which Includes all body systems & health history. (Are broad &
complete); Comprehensive screening assessment includes cognitive
and emotional development in addition to physical growth.
• Focused assessment- based on the patients’ health issues; Like
problems that are assessed by themselves. Related to the current
situation.
Framework for Heath Assessment: 3 major frameworks for organizing
assessment data
• Functional Assessment- focuses on the functional patterns that all
humans share; health perception and health management, activity,
and exercise, nutrition and metabolism, elimination, sleep and rest,
cognition, and perception, self—perception, and self-concept, roles
and relationships, coping and stress tolerance, sexuality, and
reproduction, and values and beliefs.
• Head-To-Toe Assessment: is the most organized system for gathering
comprehensive physical data.
• Body System Approach: is a logical tool for organizing data when
documenting and communicating
findings. This method promotes critical thinking and allows you to

The nursing process is a systematic problem-solving approach to identifying
and treating human responses to actual or potential health difficulties.
• Assessing the patient, analyzing data, and making nursing
diagnoses, determining patient outcomes, or planning care,
implementing, and then evaluating the patient’s status to
determine whether interventions were effective.
Data Collection:
Subjective data- The primary source in subjective data collection is the
patient. Subjective data are based on the patients’ experiences and
perceptions. Subjective data is anything the patient tells you, pts labs, urgent
assessment, and risk factors.
Objective data- is measurable data, screenings, what we can physically do etc.
Is what you can measure. (You observe the patients general appearance;
assess vital signs; listen to the heart, lungs, and abdomen; and assess
peripheral circulation.
