This care coordination plan is essential to ensure when a patient has been diagnosed with epilepsy, their life is minimally impacted. When there is an abnormally excessive or synchronous neuronal activity in the brain, it is diagnosed as epilepsy (Bastos & Cross, 2020). These abnormal activities can cause obstacles to have a functional life. The goal of a coordination plan for these patients would be to limit these obstacles as much as possible. Some of the obstacles can be these patients are unable to drive due to legal constraints or maintain a job because of the stigma and the fear of having a seizure. Health Issues Epilepsy often leads to the feeling of social isolation because these patients feel as though they are outcasts. This can lead to a delay in treatment because these patients feel ashamed of their diagnosis (Kuehn, 2019). The feeling of social isolation can lead to depression, which can impact the quality of life for these patients. Epilepsy is strongly associated with a range of mental comorbidities, which can affect as much as 50% of epilepsy patients (Elger et al., 2017). What’s more, mental disorders are one of the focuses of Healthy People 2030. When someone is experience a mental disorder like depression, it can impact their ability to take park in healthy behaviors (Healthy People 2030, n.d.). Furthermore, when someone has a physical health problem like epilepsy, it can make it harder for them to get treatment for a mental disorder (Healthy People 2030, n.d.).
