Patient Name: Bernadette Jackson
Bernadette Jackson is an 85 year old African American woman who was admitted for urinary retention and a UTI
Goals:
• Performs safe patient care using DCN protocol (CO 4, 5, 8)
• Performs a head to toe assessment and recognizes normal and abnormal findings (APPLYING) (CO 2, 7, 8)
• Designs an individualized nursing plan of care based on subjective/objective assessment data (CREATING) (CO
2, 3, 5, 6, 7, 8)
• Performs safe oral medication administration (APPLYING) (CO 4, 7, 8)

•Demonstrate team-work and collaboration skills, including delegation (APPLYING)(CO1, 4, 6, 7)
•Evaluates the patient’s responses to nursing interventions (EVALUATING) (CO 1, 3, 7, 8)
Prior to simulation review:

Please answer the following questions prior to simulation day:
1. Describe risk factors/pathophysiology/concerns for urinary tract infection in elderly patients.
a. Risk factors
i. Confusion incontinence, falls, slight elevation of body temp. dysuria, frequency, urgency.
b. Concerns
i. Pneumonia: tachycardia, tachypnea, and confusion with decreased appetite and
functioning, without the more common symptoms of fever and productive
cough
c. Pathophysiology
i. Variations from the usual norms for laboratory values are sometimes caused by
age- related changes in cardiac, pulmonary, renal, and metabolic function.
2. List 3 patient education points for UTI prevention
a. After eliminating wipe front to back, not back to front. Especially important in females.
b. Teach the client to recognize symptoms of UTI, such as dysuria that crescendos as the
bladder nears complete evacuation.
c. Urinate at appropriate intervals. Do not ignore need to void, which can result in stasis of urine.
3. List 6 causes for acute confusion in an elderly person?
a. Impaired Vision or Hearing
b. Drug toxicity
c. Serious infections
