Name: Sherman Tremaine (ST)
Gender: Male
Age: 53 Years
Chief Complaint: "I was living, and not bothering anyone, and those people—those people,
they just won’t leave me alone."
History of Presenting Illness: ST, a 53 year old male patient, is in the clinic for psychiatric
evaluation appointment following her sister’s insistence. He is a known diabetic on
metformin. The patient reports that he was living with his mother, but then she passed away
three years ago. As a result, he was left alone whereby he reports that he was living without
bothering anyone but some people won’t leave him alone. He claims that the people in
question usually stay outside his window and watch him. He reports that he normally hears
them and sees their shadows too. He claims that those people think that he cannot notice their
presence but he does. He speculates that they were sent by the government to watch him
because his taxes are extremely high, so high that they get to the sky. While giving this
history the patient also sees a bird in the clinician’s office.
ST reports that he has been seeing and hearing those people for several weeks. While
answering this, he also hears heavy metal music, and reports that whoever is playing the
music has the intention of making them believe that it is weak yet it is heavy. In addition to
the visual and auditory hallucinations, ST also reports trouble with his sleep. He says that he
tries to sleep but the voices that he hears are too loud to the extent of keeping him up for days
on end. As a result, he tries to watch the television, but the people who have been watching
him, watch him through the screen instead. He claims that those people then come into his
house and poison his food. He has, however, managed to trick them in that he locks up
