Identifying Information
Timothy Childers is a 15-year-old, right handed, heterosexual male who seems to be
likeable and may make good impressions on others. Timothy loves hanging out with his friends
and gets along well with his siblings (youngest of three siblings). He takes dance class and plays
the drums in his band at school. He is in his ninth year of education and is hedonistic and self-
centered. He also engages in risk-taking behaviors and tends towards exhibitionism. However,
he has suggested that he has a need to succeed in life. His future goals are to become a racecar
driver or play in a professional band as a drummer. His ethnicity is of a ‘mixed race’ with having
Anishinaabe, Scottish, Irish, and French origins. Yet his mother accredited that he is of
Caucasian decent.
Reason for Referral
According to Timothy’s MMPI-A evaluation, it indicated there is a good indication of
further evaluation, even with his open cooperation to become a testee for the assessment. The
assessment also indicated severe behavioral problems. More information is needed to assess a
definitive diagnosis. Furthermore, an assessment plan on how to deal with shaping his behavior
is needed. Evaluation of suicidal or harm to self should be evaluated as well. Harris-Lingoes
Subscales suggest moderate depression factors, high t scores of physical malfunctioning, 69 T
score of social alienation with a 60 probability of authority problems (Butcher, 2014, p. 8-10).
An individualized assessment into possible ADHD tendencies is probable in diagnosis of
behavioral issues.
According to the Behavioral Neurotherapy Clinic, behavioral symptoms with ADHD type
symptoms should follow evaluation or re-evaluation of individual based on the DSM-IV
behavioral criteria assessment (para.9-19). Also, according to the Diagnostic and Statistical
Manual of Mental Disorders (DSM-IV) of the American Psychiatric Association, ADHD is a
diagnosis applied to children and adults who consistently display certain characteristic behaviors
over a period of time. Therefore, an evaluation is essential at this time.
Current Symptoms/ Presenting Concerns
Timothy has significant issues with behavioral problems such as following directions,
being disruptive and loud, and has authoritive issues. He is argumentative at school and in his
home environment. Timothy has a discord for following rules unless they are in his favor, and is
very impulsive at times. He displays a lack of empathy, is self-centered in the aspect of always
wanting to be the center of attention or talking about himself, and always disrupting
conversations without a lack of consideration. Timothy hates getting in trouble but continues to
defy all authoritive figures at school and home. He may try to harm himself or runaway due to
aspects of low self-esteem issues that have stemmed from a sense of not being good enough.
Timothy also has relationship issues, in the sense that he sees relationship in a negative aspect,
and there is some suggestion that he tends to worry too much about insignificant events.
