Identify the topic and the nursing practice issue related to this topic. (This MUST involve a nursing
practice issue.)
I have chosen frequent falls among elderly patients in the hospital. As a nurse it is my responsibility to
keep my patient safe from harm and when a patient falls there is a crack in the system to keep them safe.
A fall can drastically change the patients’ level of functioning and their quality of life.
Briefly describe your rationale for your topic selection. Include the scope of the issue/problem.
I feel that falls among the elderly is a common problem in many facilities. There are many reasons that
elderly patients fall in the hospital, from altered mental status to chronic health conditions that hinder the
patients balance which could cause the fall or maybe the room is cluttered, and they have fallen over a
blanket. There can be many reasons that they fall.

Star Point 2: Summary (Evidence to support need for a change)
Describe the practice problem in your own words and formulate your PICOT question.
Would early physical therapy to elderly patients on the medical surgical unit decrease the number
of falls that they may have.
In inpatient medical elderly patients, would early physical therapy, compared to no therapy plan,
decrease the amount of falls by 50%, within a 6-month time frame.

elderly falls in the hospital and what the general causes are.

program: delirium, low body mass index, and other risk factors. Clinical Interventions in Aging,

1253-1261.




Briefly summarize the main findings (in your own words) from the systematic review and the
strength of the evidence.
Both articles are regarding elderly patients and frequent falls. One discusses the reasons that the patient
may fall, and my main article examines early rehabilitation in hospitalized elderly patients and the effects
it has on the functional outcome during their hospital visit and the exercises that they will be doing.
Outline one or two evidence-based solutions you will consider for the trial project.
Incorporating daily exercise routine for elderly patients to increase their strength to help decrease their
hospital stay.

Identify care standards, practice guidelines, or protocols that may be in place to support your
intervention planning (These may come from your organization or from the other sources listed in

Patients need to be kept safe, by following the fall precautions set in place this can be accomplished.
Following the fall precaution guidelines is necessary to have all precautions in place. Make sure no clutter
in patient room, proper lighting is on, bed alarm is in place.
