Personal Wellness Plan – Submit through Brightspace
Throughout this course, you have been evaluating your own personal health habits and behaviors.
This assignment gives you the opportunity to evaluate your current health behaviors, identify areas of
concern and set goals for improvement. In each chapter, you will notice that there are labs that we
did not do that allow you to set goals for the various components of health. For example: You
reflected your overall assessment of your personal wellbeing with the Personal Wellness Wheel in
Chapter 1. In other chapters we discuss the importance of cardiorespiratory fitness, improving
flexibility, muscle endurance and strength, eating nutritious, maintain a healthy weight, decreasing
stress in your life, mental health, and health relationships. You can/should refer to these for
assistance in completing this assignment.
Your personal wellness plan should include the following:
1. A brief synopsis/overview of your personal health. Be sure to address positives as well as
areas where improvement is needed. This section should be based upon the information
gathered during the labs for each chapter. (20 points)
a. Be sure to address the following areas: cardiovascular, muscle endurance and
strength, flexibility, body composition, mental health, spiritual health and nutrition.
(350-400 words)
2. Choose AT LEAST 2 different areas where you could improve your health and write a
minimum of 1 short term and 1 long term goal for each area. (4 goals total) (5 points per
goal/20 points total)
a. When writing goals remember to follow the SMART guideline. (Specific,
Measureable, Action-oriented, Realistic and Time Oriented) You can refer to your
textbook for examples
b. With each goal, discuss briefly obstacles that might hinder your plan AND strategies
you might use to address these obstacles. (125-150 words / goal)
