Task 1: Health Assessment
A. Include two completed copies (name, address, phone, date, and signature) of the attached
“Nursing Program Audio/Video Recording Consent and Release Agreement,” one for yourself
and one for the friend/family member acting as your patient for this assessment.
B. Record yourself completing a comprehensive health assessment using the Panopto system
by doing the following:
C. Include a completed copy of the attached “Comprehensive Health Assessment
Documentation Form.”
D. Analyze (suggested length of 1 page) the primary findings of the comprehensive health
assessment you performed.
The patient, S.P., is a 25-year-old Caucasian/Hispanic female. She is 5’7” and weighs 180
pounds. She presents today seeking to become an established patient and get a prescription for
blood pressure medication. Upon assessment her vital signs are BP 133/63, HR 86, RR 16,
T 97.9, O2 Sat 100% room air. She is awake, alert, oriented, appropriate and cooperative. Her
lung sounds are clear in all lobes, no adventitious lung sounds. Respirations are even and
regular. No spontaneous cough. Her heart sounds are regular with a full apical pulse. S1 and
S2 have a regular rhythm. Bilateral palpable pulses are noted. Capillary refill is <3 seconds.
No neck vein distention noted. No edema noted. Her BMI is 28. Her general appearance is
healthy but her BMI suggests overweight. Her surgical history is a tonsillectomy at age 5. She
has a 3-year-old daughter. Her pregnancy and birth were uneventful.
1. Identify lab tests that should be ordered based on your assessment findings.
The blood tests I will order are complete blood count (CBC), comprehensive metabolic
panel (CMP), and Hemoglobin A1C (HbA1C).
