, 1.The patient is a 58-year-old woman with chronic pain due to
inflammatory arthritis. She presents for her first appointment with
you in a primary care office. She states that she is aware that she is
asking for an early refill of her Oxycontin however she is traveling out
of state, and she is concerned that she may have a flare-up on her
trip.

Screening, Brief Intervention, and Referral to Treatment (SBIRT) are used to
identify and deliver services to those at risk for substance use disorders,
depression, and other mental health conditions (Hargraves et al., 2017).
SBIRT is an effective tool that can be used to empower primary care
providers to help identify and treat patients with substance use and mental
health problems before costly symptoms emerge (Hargraves et al., 2017).
As primary care providers, we can identify and intervene early (Hargraves et
al., 2017). Educating the patient on the risk of opioid addiction will be
important in this scenario. Up to 25 percent of patients receiving long-term
opioid therapy in a primary care setting struggle with addiction (Torres et
al., 2017). Determining if patients have been exposed to narcotics in the
past and whether they have developed a tolerance for narcotics is helpful in
establishing a patient/provider relationship with trust, as well as determines
the potential for abuse, and or misuse, or overdose (Torres et al., 2017).
Using a Drug Screening Questionnaire (DAST) can help determine if the
patient would benefit from a reduction, education, or referral to receive
