Identifying Data
Date of Initial Assessment: 2/14/2021 PSEUDO Name: Jamal Malik
Reason for Referral/Presenting Problem
Jamal came to the Government Community Service Center for an assessment by direct
referral from a staff of the Center for Spiritual Enlightenment. The referring Spiritual Advisor at
the Center for Spiritual Enlightenment, having a master’s degree in counseling, believed Jamal’s
symptoms were described as PTSD in opposed to dhat syndrome that he was diagnosed with at
the Center for Spiritual Enlightenment. Reportedly, by Jamal’s brother, after Jamal having had
appeared on the American Game show, who wants to be a millionaire, 15 days ago, Jamal has
been in a daze almost all day, not sleeping at night, and having nightmares. Jamal’s brother also
reported that Jamal was easily startled and refuses to go anywhere near the Mumbai jail. Jamal
self-reported that he did not remember anything from the game show he appeared on. Jamal
further reported that it felt as if the city around him was no longer real. Jamal further reported
that he feels like a detached observer of his behavior and he is watching the world bump along.
Source of Information. The information provided was collected initially from notes
released from the Center for Spiritual Enlightenment intake at the time of referral, statements
made by Jamal’s brother, and Jamal self-reporting. Additional information provided, was from
the self- reporting measurement of the Acute Stress Disorder Scale that was administered to
Jamal. The Acute Stress Disorder Scale was helpful in ruling out ASD and confirming Jamal’s
PTSD symptoms.
