Week 5 RUA: Health History
Demographic Data
BK is a 47-year old Asian Indian female. She was born in Amritsar, Punjab, India. Patient
is one of nine siblings, one of which passed away. Both of her parents have also passed. She is
the youngest daughter and all of her siblings reside in Punjab, India. BK graduated college with
her B.A. from India. She got married and came to Sacramento, California. Together with her
husband, she has one daughter and one son. Patient works as a state office assistant. She loves
her job and has been given many shout outs regarding her excellent work.
Perception of Health
To BK, being healthy means to be happy mentally and emotionally, and to have wellness
physically. She is happy right now with her occupation and her family. As far as her body’s
wellness, she feels that she needs an overall wellness checkup. She would like to lose 15 pounds

and take care of her body.
Past Medical History
Patient does not smoke or drink. She has no known allergies. BK has chronic migraines
for which she takes prescribed medication occasionally. She occasionally Tylenol for body
aches/tiredness after a long day of work or cleaning. Besides that, she only takes hair, skin, and
nail vitamins daily. BK had a bunionectomy for her right foot in 2018. Her immunizations are
up to date. BK sometimes has ulcers in her stomach, which her doctor is aware of. However, she
does not take medications for them.
Family Medical History
Mother and father and middle brother passed away from a heart attack. Oldest sister is
also a heart patient. Second oldest sister has osteoarthritis. Oldest brother is a chronic smoker
