Introduction
This case study is about Mr. M a 70-year-old man that lives in an assisted living facility.
His health condition has a sudden decline in the past 2 months. This got the attention of doctors
to get him to order testing. He is having memory issues not remembering simple things like
family members or even room numbers. Besides, his labs report that some high results are
impacting his health. Therefore, some interventions can help him and his family members to
have a better quality of life.
Clinical Manifestation
Mr. M presents with many clinical manifestations. Mr M is 70 years old living in an
assisted living facility. His previous medical history includes hypertension controlled with ACE
inhibitors, hypercholesterolemia, status post appendectomy, and tibial fracture status postsurgical
repair with no obvious signs of complications. Over the past two months he has a sudden decline
in his health and limited physical activities. Mr. M present with memory loss as it shows signs of
not remembering names, what he has just read, or even his room number. Mr. M also shows
signs of aggression, fear, and agitation. On the other hand, some objective findings where he
measures 69.5 inches, weight 87kg this being a little high as his BMI is 30, his temperature is
normal at 37.1 C . Mr. M on labs shows high lymphocytes of 6700 (cells/uL). His blood pressure
was 123/78 HR 93 RR 22 and Pox 99% (Lange, 2016).
Primary and Secondary Medical Diagnosis
Based on the information presented, the primary diagnosis should be high blood pressure
by the objective data. Urinary tract infection is another one as the urine tested positive for the
moderate number of leukocytes and it was cloudy and also the WBC was elevated. A secondary
medical diagnosis can be Alzheimer's disease this is shown by fear, aggression, and agitation and
