Introduction
Assessment is a method to define the nature of that problem, to determine a diagnosis,
and to develop specific recommendations for treatment to address the problem or diagnosis.
Health Assessment is the collection of data about an individual's health state (Jarvis, 2008).

Health Assessment of patients could provide a more holistic approach by integrating some of the
techniques. When assessing the health of an individual it is important to consider the entire
picture of the patient. Therefore, it is vital to have assessment skills perfected to the highest
level as the nurse is the only health care professional in the hospital who is continuously
present at the bedside. Health assessment includes factors such as; culture, values, family, and
support networks and not just the physical being, as well as the ability of the patient to care for
self.
These factors must be assessed in order to follow a holistic model of care (Jarvis, 2008).
Reflection Experience
A holistic health assessment allows the nurse to obtain essential information for
diagnosis, planning, and implementation This shows respect for the preferences of the patient,
which preserves the dignity of the patient. Physiological, psychological, sociological,
developmental, spiritual, and cultural are the six aspects of holistic assessment. Integrating the
six elements of a clinical evaluation into the nursing assessment will help identify a root cause of
the disease or hinder healing by other causes. Nurses can use this method to glean information
about the circumstances, traditions, attitudes and other aspects of the patient's family that would
influence the treatment given and the recovery of the patient. This could lead to more efficient
care and optimum health. Physical examination techniques and collecting history are a time-
honored skill of healing and patient care. Collecting sensitive and personal information and
performing detailed and accurate examination strengthens our relationship with the patient,
guides our assessment, and sets out the criteria for our clinical decision-making. Most people
view physical examination and health assessments with some anxiety due to exposure to their
body part, potential pain and discomfort with the findings of the physician. As a bedside
