place. To prevent such accidents, having a fall protection system in place will eliminate
the hazard of falling to avoid major accidents. Lack of risk assessment is one of the
underlying factors in the accident. The working environment has an ineffective safe
work procedure, placing warning signs, protective safeguards, and proper lighting to see
the danger might prevent the accident. Lack of communication, every worker should be
aware of all the possible hazards in the working area, no one should be left unaware of
