General Objectives: To administer assessment of skin, hair and nail with maximum
comfort and safety for the patient and nurse.
Specific Objectives:
1. Conduct accurate history taking of the client’s skin, hair and nail condition.
2. Perform physical assessment of the integumentary system using the correct method
of inspection and palpation.
3. Differentiate between normal and abnormal findings.
4. Describe the findings frequently seen when assessing older patients.
5. Analyze the data gathered from the interview and physical assessment and
formulate valid nursing diagnosis, collaborative problems and/or referrals.
