Executive Summary
There may be occurrence of adverse events during patient interaction with the healthcare
system. Regarding adverse drug events in the health care system, hospitalized patients stands
between 5% and 20% while hospital admissions related the adverse drug events stands at 3% to
14%. Regarding adverse events in associated with infections such as malaria and HIV, the
medical devices errors responsible for these infections include hypodermic syringes, needles, and
blood products. 28% of the admitted patients acquire these hospital infections based on the errors
named above. This relates to the unsafety culture of hospital while providing health care [1].
The existing policy options include staffing nurse for healthcare, health consumers
empowerment, and medication review in health facilities. Eventhogh nurses are responsible in
reduction of deaths against hospitalized patients as well as reduction of the stay duration among
admitted patients, they are also responsible for low readmission of patients due to their errors
while providing healthcare. There is lack of sufficient consumer involvement during creation of
healthcare research and policy, patient information, and guidelines for practice. Inappropriate
prescribing can be reduced by review of medication to minimize adverse drug events such as
low-quality drug adherence and drug interactions. This may help reduce hospital emergencies as
well as hospital mortality. In order to implement the proposed policy effectively, there need to be
mobilization and sensitization of the community so as to equip the relevant stakeholders with
skills, knowledge and motivation [2]. There need to be training of healthcare professionals,
especially those from facilities that lack pharmacists to help with drug management and
administration. In addition, there need to be motivation of health professionals especially in
marginalized areas.
