Professional Capstone and Practicum: Reflective Journal
Week 1 (04/10/17)
Day one: Here I am wondering what this course, The Professional Capstone, and
Practicum will involve for the next ten weeks. I understand that this is like an internship program
which enables me to apply all that I have been learning in my previous classes into the real world
with the assistance of a mentor. I have read all the instructions and requirements of this course,
and I have scheduled to meet with my mentor to review them and complete the Individual
Success Plan (ISP).
Day two: I seem to be lost about what exactly I am supposed to accomplish for the week.
I spoke to my instructor to get a better understanding of how to complete the document. I was
elated when she not only explained the process but also gave the class an example. I am sure that
everyone will benefit from this example because the other students may very well be confused
too. I completed the ISP with my Mentor and now we are off to the next step which is choosing
a project topic. After several deliberations, I have decided to write on the issue of medication
compliance among the elderly home care patients. Currently working on my PICOT question.
In this project, I am understanding more and more how patient care delivery has evolved
over the years through the incorporation of evidence based approaches to care. Nurses continue
to be at the forefront of healthcare transformation into a system that adopts the use of new
knowledge through evidence based practices to bring about a more refined patient outcome
(Stevens, 2013). So far, I am getting stronger in my profession, gaining more confidence and
having a stronger voice in my workplace when interacting with other professionals, knowing that
I can make a difference in a patient’s life by acquiring more skills and knowledge through
education and competency and advocating for change to evidence based practices (EBP).
