, The following document is to be used as a guide to detail the response and
approach that will be taken to address the citations and penalties that will be taken to
address the findings from the OSHA inspection.
The initial steps that should be taken immediately is to pay the proposed
penalties that were identified to reduce the total cost in interest. All of the immediate
corrections shall be made today, and implemented immediately. The third party safety
consultant, and industrial hygienist need to be called immediately to begin the process
of developing and implementing procedures and assisting in sampling of the workplace.
Citation 1 Item 1a-1c Exposure levels to lead fumes to welders.
1. Pay the proposed penalty of $7000.00
2. Provide the employees with effective respirators immediately as an interim measure.
3. Modify the current ventilation system to reduce the exposure of lead below the
PEL to the welders.
4. Modify the ventilation hoods to collect the metal fumes more efficient.
5. Develop a detailed abatement plan to the OSHA area director addressing the
proposed requirements detailed in the citation document. This document shall
be completed no later than 09/01/16.
6. After completion of ventilation system upgrades, air sampling needs to be implemented
to ensure the newly modified ventilation is adequate.
Citation 1 Item 2- Respiratory protection program.
1. Pay the proposed penalty of $4900.00
2. Third party safety consultant to develop and implement a respiratory protection program
that established procedures and requirements for workers exposed to metal fumes.
3. Develop an abatement document to address the violations no later than
09/28/2016. Citation 1 Item 3- Medical Evaluations
1. Pay the proposed penalty of $3500.00.
2. Contact medical facility to provide medical questionnaire for employees that
are in the RPP.
