Hypothyroidism: Planning Care for a Client Who Has Hypothyroidism (RM AMS PN 10.0 Chp. 71 Hypothyroidism) o Laboratory tests:  T3 expected reference range:  70-205 ng/dL in clients age 20-50  40-180 ng/dL in clients older than 50  T4 expected reference range:  4-12 mcg/dL in male clients  5-12 mcg/dL in female clients  Expected with hypothyroidism'  T3 decreased o Patient- centered care  Monitor for cardiovascular changes ( low blood pressure, bradycardia, dysrhythmias.)  Monitor the client's weight. Inspect for peripheral edema.  If mental status compromise occurs, orient the client periodically and provide safety measures.(Qs)  Increase the client's activity level gradually, and provide frequent rest periods to avoid fatigue and decrease myocardial oxygen demands.  Apply antiembolism stockings, and elevate the clients legs to assist venous return.  Monitor respiratory status including rate, depth, pattern, oximetry, and arterial blood gases. Encourage the client to cough and breathe deeply to prevent pulmonary complications.  Consult with dietitian. Provide a low- calorie, high-bulk diet, and encourage fluids and activity to prevent constipation and promote weight loss. Administer cathartics and stool softeners as needed. Avoid fiber laxatives, which interfere with absorption of levothyroxine. (Qtc)  Provide meticulous skin care. Turn and reposition the client every 2hr if the client is on extended bed rest. Use alcohol-free skin care products and an emollient lotion after bathing.
