CHIEF COMPLAINTS
Mr. F, 42 years old gentleman with no known medical illness presented to the
hospital due to loss of vision for 1 day prior to admission.
Or RE redness and discomfort for 3 days duration
HISTORY OF PRESENTING ILLNESS
At work, 3 days prior to admission,
Working with air jet to clean machineries (solar wafer), exposure of eyes with
contact lenses to dust.
Felt discomfort that leads him to clean his eyes immediately with a water
sprinkler.
Leave contact lens on for the next 2 hours until he noticed some discomfort over
the right eye.
Back at home,
Right periorbital swelling & red right eye
Onset: Day 1 of illness, noticed as soon as he arrived home from work
Progressive & persistent
Associated with minimal redness, itchiness, increased tearing and watery
right eye
Early course of disease
Tenderness on touch
Minimal pain of right eyes
Site: RE, anterior eye, posterior eye
Onset: Day 1, pain is perceived as he arrived home from work, sudden
onset
Character: Striking sharp pain anteriorly, constant and non-progressive
Radiation: Throbbing/pulsating sensation at the back of the head
Associated symptoms: Swelling, Excessive tearing, Itchiness,
Grittiness, Warmth over eyes
Timing: No diurnal variation
Exaggerating symptoms: Pain on movement of eyeball (Foreign
sensation in the eye), Pain on exposure to lights including LED
(Photophobia)
Severity: 4-6 pain score, most painful if exposed to lights and LED/
movement of eyeball
Day 2 of illness
Received medical attention from private clinic
