Management of Complex Problems of Adults/ Older Adults LEARNING ASSIGNMENT FOR PERIPHERAL VASCULAR DISEASE Individual Case Study: Andrew Stone is a 63-year-old Caucasian male who presents with pain in his right calf. The pain began several weeks ago when he began a walking program to incorporate more exercise into his routine. Although, this is Mr. Stone's first visit to your office, he is scheduled for a 15-minute visit. CC: Right leg pain with walking HPI: Pain occurs in the right calf and began approximately 1-2 weeks ago. Cramping in nature, and at its’ worst is an 8/10; sometimes both legs feel heavy. The pain comes on with walking and improves with rest. After sitting for five to ten minutes, he can resume walking for another 10-15 minutes, but then the symptoms reoccur. He is concerned because the pain starts to bother him after walking only 3-4 blocks. He denies loss of sensation or motor weakness in his legs, nocturnal symptoms, swelling, or color or temperature changes in his legs. He has no other problems or complaints today. PMH: Hypertension; denies diabetes or heart disease. PSH: Denies FH: Maternal grandparents died “in their 80’s due to old age” Paternal grandparents- history unknown Mother, died at age 81 of breast cancer Father, alive, age 88, history of MI, DM, Hyperlipidemia Brother, alive, age 60, history unknown Sister, deceased at 41 due to an MVA SH: He is a retired police officer and spends his days working on model ships. He has been married for 28 years. He and his wife enjoy attending social events together. He smokes 1/2 pack of cigarettes per day and recently cut down from smoking 2 packs daily for the past 30 years. He has a history of alcohol abuse with five years abstinence. He denies use of recreational drugs. He has recently started a low-fat diet and a daily walking program to lose weight. Meds: HCTZ 25 mg daily. Multivitamin daily Allergies: NKDA
