Clinician: Let's get you seated so I can take your vitals. Fred: Okay. Clinician: Takes vitals and notes them on his chart Did you remember to take your medicine when you woke up Fred? Fred: Yes I did. Clinician: Awesome, well everything in your vitals looks normal! Alright, I'm going to put this bib on you so that we don't get your shirt all wet! We don't want that, right? Fred: No. Clinician: Also, here are some glasses you can put on that will protect your eyes. Would you like clear or dark ones? Fred: The dark ones. Clinician: Would you also like for me to put some music on? Fred: Yes please. Clinician: What kind do you like? Fred: Rock. Clinician: How cool, so do I! Turn on the radio; music has a calming effect Alright mister, I'm going to lean you back so that I can clean your teeth and get them all nice and shiny. You're going to hear some funny sounds, but make sure to keep your arms by your sides with your hands in your pockets. Fred: Yes, okay. Clinician: Do cleaning and gentle root planing, making sure to give him a couple of breaks in between so that he doesn't become antsy and start to move around. Also pay careful attention to his gum line so that you don't cause anymore inflammation than there already is. All done with the cleaning, how do they look? Fred: They look pretty! Clinician: Yes they do! You have good teeth and we need to keep them like that! How do you clean your teeth at home? Fred: With a tooth brush. Clinician: Do you use a regular one or an electric one like this? Shows him an electric toothbrush Fred: A regular one. Clinician: I think it would be really great if you got an electric toothbrush because it does a much better job of keeping your teeth clean and it feels really cool against your teeth! Let me show you how it works and how it feels against your hand. Make sure you don't press too hard or else it could hurt your gums. Gently put the toothbrush against his skin so that he can feel the slight pressure and show him the correct motion to use Fred: Okay...oh it does feel cool. Clinician: See I told you! Do you floss Fred? Fred: Not really. Clinician: Well your teeth are very easy to floss and it'll help keep them clean and healthy. Because he has Microdontia, his teeth are spaced out With these floss picks, it is really easy to get back into those hard to reach places. Here, let me show you how. Demonstrates using floss pick on him and myself I need you to brush and floss in the morning and at night before you go to bed. Can you give that a shot? Fred: I can do that. Clinician: You're awesome! Now one more thing Fred, I noticed that your mouth is a little dry, does that bother you sometimes? Fred: Yeah it does, so I just drink more juice. Clinician: I thought so. What I would like you to do is to switch that juice for water and use a Biotene rinse after you brush and floss. Here's a bottle just for you! This will help a lot with your dry mouth and it'll help make your gums feel a lot better too. Fred: Okay, thank you. Clinician: Alright big guy, I want to see you again in 3-4 months to keep these teeth of yours in tip top shape, sound good? Fred: Sounds good! Clinician: Thanks again for coming today Fred, see you soon! For example: Clinician: Good morning, Fred. how are you feeling today? Fred: Good Fred doesn't have to say much, you will do most of the talking. Remember be specific and address his chief complaint, along with other important data that the patient should know? Clinician: (I would begin in the waiting room ) how are you ding today fred? I'm so happy that you have come by to see me again. I always enjoy our planned visits. Fred: Im happy to be here. I cant wait to get that new teeth brush at the end. Clinician: Of course fred, I have set aside you favorite color just for you. Fred: thank you Clinician: Follow me this way fred. Would your friend like to come back with you. Fred: yes please, I like my friend to be with me. Clinician: am going to begin by having you take a seat in this cozy chair. Fred: there Clinician: yes there. Fred: this is cozy Clinician: let me begin by putting on this bib and cool sunglasses. Fred: puts on the glasses Clinician: first let me start by looking at your tooth pictures. I call them x-ays. X-rays are very neat becouse they see all the way inside your mouth. They even see you bone! Fred: My bone? Clinician: yes your bone. Fred: what do you see. Clanician: I see many things. To begin with, I can see that you have a little bone loss. Your teeth are also a little small. Fred: why does that happen? Clinician: I think it is your thyroid medication is causing the bone loss. Your teeth are small just because you where bone that way. I am going to begin my laying you back. Your gums also look a little puffy . That is probably because I need to teach you how to clean your teeth a little bit better. ( I show fred how he should be brushing and flossing. I repeat the instructions several times. I have him show me and repeat the instructions). Fred: wow I didn't know I was supose to be putting string in between my teeth. I will start to do this every day from now on. Clinician: Now I am going to start off cleaning your teeth .Your going to hear different sounds that's just going to me trying to get your teeth nice and clean. If you feel you need a break I would like you to raise you hand a bit. (stopping often for him to relax). Fred: ok I'm ready Clinician: after I finish cleaning freds teeth I have him do a flouride raise. I tell him that he can not eat or drink anything for thirthy minutes. I let him know that I am going to give him a producuct to help him with his dry mouth . I tell him he needs to use it every day and that will help him. I explain that when your mouth is so dry its easier for all the bad grems to hide in his mouh. We want to keep bad grems out. Fred :that wasn't so bad. Clinician: I'm glad to hear that. We will be seeing each other often. I would like to see you back here in my office for another special cleaning in three months. When I see you back I want you to show me how good your doing with your flossing and burshing. So are you happy that we got you teeth nice and clean like you wanted Fred:Yes, thank you Clinician: you welcome. Now let me set you up and give you that new tooth brush that I promiseed you. Now lets go up front to talk to my friend maggie. Maggie will make sure to schedule your next appiont in the morning just like you like. Fred : thanks again bud.
