Prioritization
Prioritization- a process that envisions client outcomes
• When you are deciding about which should come first you are predicting an advance of
which one is needed b/c you are expecting an outcome
• Predicting problems based upon the choices that are available and weighing any future events
• Making decisions clinically you are always prioritizing
• Must have knowledge about disease in order to prioritize and take care of patient based on
a whole bunch of stuf
• Triage- used in emergency medicine as a way of sorting patients into most critical or stable,
who needs something the most and you can wait a while
• Ways to prioritize:
▪ ABC
▪ Vital signs
▪ Past history to predict the future
▪ Length of time that will be required for care of the day: ex dressing change could afect
prioritization of the day
▪ Maslow’s hierarchy of needs
▪ Patient situations are dynamic b/c what you get in report is probably a reflection from
15 minutes or 2 hours ago and things can change quickly from report to current or from
bed
1 to bed 2
▪ Stable vs unstable, Predictable vs unpredictable, Known vs unknown

• Post-op patient would be higher if they haven’t been evaluated yet b/c their condition
is unknown
• Patients who are admitted usually go bad within 24 hours of admission; a new admission is
high priority than someone who has been admitted for a couple of days as long as their VS are
stable
• Have to make a decision on which needs or problems require attention immediately, which
can be postponed and for how long
• In order to prioritize safely and accurately you need to know:
▪ the purpose of the care or activity
▪ current status where we are, where we want to go and what you want to achieve
▪ what team members you have to rely on and what their role is
▪ First priority: Life threatening, is the person likely to die if this is not done, will someone
else be in danger if I tend to this task, is it essential to patient or unit safety and is it
essential for nursing care. Asepsis is essential
• Levels of prioritization- based on Maslow’s hierarchy
1. First: ABC’s, vital signs, chest pain (b/c predict cardiac death if not addressed)
2. Second: altered mental status changes (confusion), untreated medical issues, acute pain
(not chest pain related), acute elimination problems, abnormal labs and any risk
3. Third: education, rest, coping, support and self-esteem
• Family & patient preference and request when possible, but they don’t take priority
• As a guide for prioritization use the nurse practice act, ANA code of ethics, statue of nursing
practice & policy and procedures for job description. Example if a post-op patient is
ambulating
