Family-Centered Health Promotion
Family-centered health promotion has come to the forefront of today’s healthcare. To
better achieve individual health and wellness, the nurse must conduct an assessment and
evaluation of the family. The family and individual are intertwined to act and react with each
other to determine potential for behavioral change to promote wellness. Social determinants of
health (SDOH) influence individual and family health (Falkner, 2018). Various health models
have been recognized over time that provide a framework for understanding patient’s health
(Falkner, 2018). Nurses must conduct a family-focused assessment to determine patient needs
and develop a plan of care to elicit change in behavior to increase wellness and health promotion.
Social Determinants of Health
Social determinants of health (SDOH), according to Falkner (2018) include various
factors such as the environment in which an individual lives, works, plays, learns, and worships,
as well as their socio-economic status and education. These influence an individual’s health and
well-being. Westphal (2019) states that community-based organizations have recognized that to
meet the needs of an individual, they must be met “where they are;” they must be treated in
respect to their social determinants. The interviewed family live in a middle-class rural
community. They own their home and all basic needs such as food, shelter, and clothing are
met. They have access to healthcare close to home and advanced healthcare within an hour’s
drive.
Both parents work and carry health insurance. The mother has a bachelor’s degree and the father
has a high school diploma. The children both attend public school. This family’s social
determinants of health promote wellness. They have access to and utilize their primary care
physicians. They have clean running water and access to climate-controlled housing. They have
