You are the nurse on- duty in the obstetrics out patient department and have two patients.
Alice, a 33-year-old woman is 3 months pregnant with her second child and married to her husband for
8 years. She had her 1st pregnancy when she was 26 years old. She finished her nursing degree and now
works as a hospital nurse. While Anna is a 16-year-old high school student in her 3rd month of
pregnancy and has yet to inform her parents since she is worried that they might get mad at her. She is
with her boyfriend Andrew, 16 and also worrying over the situation.
Questions:
A. As a nurse on duty, what will be your initial assessment to Anna and Alice? How do they differ in
terms of their situation?
An accurate assessment of pregnancy at the initial visit or check-up is important. In the
assessment of a pregnant woman, it is very important to determine and describe first the signs
and symptoms that will stipulate if the pregnant woman is in good health condition. In the
scenario, it is stated that Alice is a nurse and it means that she already has the knowledge and it
is now her second child so therefore, she already knows what to do and what are the things that
needs to be avoided during her pregnancy. While Anna is a 16-year-old; therefore, carries more
health risks for her and for her baby also, as a nurse I have a lot of important things to discuss
with Anna. It is important to assess and ask the pregnant women, Alice and Anna for symptoms
that might be dangerous or may suggest poor nutrition or lack of iodine in their diet because a
poor maternal nutrition is associated with poor pregnancy outcomes. I will assess an describe if
there are warning signs and symptoms that might indicate health problems that may lead to a
serious complication of their pregnancy. The next thing to do is the physical examination which
includes the general appearance, nutrition, their weight and of course the vital signs, which is
very important. Both Alice and Anna are 3 months pregnant. Screentests are needed to know
and examine if their baby is at risk of a serious health conditions. The tests are the combined
results of blood test and ultrasound to assess the risks of having a baby with Down Syndrome.
This test can also detect some abnormalities and may also tell if you have a multiple pregnancy,
for example twins.
