SBAR for Josephine Morrow
Situation This is Cianna, I am the unit nurse taking
care of Josephine Morrow. Patient is
experiencing pain in lower right leg and is
forgetful of some recent events.
Background Josephine Morrow is an 80-year-old female
with a venous stasis ulcer. Admitted 3 days
ago with a diagnosis of chronic venous
insufficiency; right lower extremity venous
stasis ulcer. Allergic to Penicillin, vital signs
are as follows BP: 124/83, HR: 90, RR: 21,
SpO2: 94%, Temp: 98. Current
medications include Aspirin, Zinc
supplement, and Multivitamin for daily use,
as well as Albuterol/ipratropium inhaler,
and Acetaminophen PRN. Abnormal lab
values include Creatinine 1.1, Total protein
5.8, Albumin 3.2, Prealbumin 18.1, See
laboratory chart for remaining labs.
Assessment Integumentary and musculoskeletal
systems are being affected. The
problem is not severe yet.
Will you order an ultrasound and X-Ray of
the lower right leg? Also, can you please
Recommendation asses the patient for pain and edema?
