Congestive heart failure (CHF) limits the heart's ability to meet the body's circulatory needs resulting in fatigue, shortness of breath, and edema. According to Bhatt et al., 2018, the prevalence of Heart failure in the US will increase by 46% by 2030. Furthermore, approximately fifty percent of patients with heart failure die five years after diagnosis (Bhatt et al., 2018). This paper summarizes the symptoms of CHF, pathological interaction between the pulmonary and cardiovascular systems, and the impact of race and ethnicity on physiological functioning. Cardiovascular and Cardiopulmonary Pathophysiologic Processes The process of cardiac structural changes due to cardiac injury and diseases like hypertension, stenosis, myocardial infarction, etcetera precedes symptoms seen in heart failure patients (Rosenthal & Burchum, 2021). Injury to the heart vessels causes less contractility and elasticity. Damage to the left ventricle, for instance, leads to the accumulation of fluids in the lung tissue causing the inability to oxygenate blood for systemic transport. In the same way, other systems, including peripheral and hepatic systems, retain fluid hence the edema, shortness of breath, ascites, and orthopnea. Cardiovascular and Cardiopulmonary Pathophysiologic Processes Interaction Due to the symbiotic relationship between the cardiovascular and the pulmonary systems, failure in one system leads to deficits in the other. Pathological changes in cardiovascular function trigger a compensatory effect by other systems to allow for adequate distribution of oxygenated blood throughout the body. The sympathetic nervous system increases heart rate, blood pressure, and afterload via vasoconstriction, while the renin-angiotensin-aldosterone system retains fluids to ease the heart's workload (McCance & Huether, 2019). Also, the necessity of oxygen in all systemic tissue functions increases cardiac workload, making simple tasks in CHF patients exhausting. Ethnic disparities exist in this population and are essential to the disease's prevalence, morbidity, and mortality. According to Breathitt et al., 2019 and Lewsey & Breathitt, 2021, CHF affects black individuals the most compared to other races. Furthermore, access to preventative care, cardiovascular treatment, and other therapies is limited in predominantly black communities (Breathett et al., 2019). There needs to be an emphasis on sensitizing people in such communities about the causative agents, preventative measures, and access to cardiovascular care. Although some factors like genetics predispose individuals to heart failure precursors, others like obesity, smoking, hypertension, and diabetes are modifiable through healthy eating, exercising, and routine medical checks. Conclusion Congestive Heart Failure pathophysiology involves the interaction of the cardiovascular and cardiopulmonary systems. Failure in one of the systems is detrimental to the other's operation. Disease and structural damage lead to vessel inefficiency and pulmonary insufficiencies leading to edema, shortness of breath, fatigue, and orthopnea. Racial and ethnic factors influence prevalence, morbidity, and mortality in CHF patients. Advanced Practice Nurses must understand the interaction of these systems and racial-ethnic variables to treat their patients better.
