Patient Information: CM, 13 yo Male, Caucasian CC: Testicular pain HPI: Patient is a Caucasian 13 y/o old male who present to the clinic with acute and sudden onset of left testicular and left groin pain started 2 hours ago s/p playing basketball and doing “lay-ups”. Pt reports constant pain of 9/10 in left scrotum reported as feeling like being “punched”. Nothing makes the left scrotum pain better or worse and no medications for pain have been attempted. Left groin pain reports as superficial/deep pain 9/10 that is getting worse. No direct impact or trauma to left groin reported. Associated symptoms include nausea with one episode of vomiting since the pain began. Nothing makes the N and V better or worse. Formal operational stage: patient’s uses symbols to relate to abstract concepts and able to make hypothesis and grasp abstract concepts and relationships. Current Medications: None Allergies: NKDA PMH: No History of major childhood illnesses. Has had regular checkups with pediatrician Tympanostomy tubes as a young child No hospitalizations, surgeries, ER visits, or specialist provider visits since last well visit. Immunizations up to date for age. FMH: Father: age 49; hypertension Mother: age 48; no medical problems Siblings: sister age 9; healthy Spouse/offspring: N/A SH: Marital status/personal support system: dependent adolescent Housing: lives in a single-family home with sister and both parents Occupation: middle-school student Substance use (tobacco, alcohol, other substances): none Pertinent sexual history: not sexually active History Questions for HPI, PMH, ROS...
