Introduction to Clinical Care Coordination




When asked how important leadership skills are to being an effective nurse, many people may
have to think about their answer. After all, what the patient sees is the people who provide their
care, not the coordination of care that occurs outside the examination or hospital room. Whether
working in a hospital, extended care facility, clinic, or health care provider’s office, nurses are
the team members responsible for managing the care provided to the patient with whom they are
working.

• By completing the Clinical Care Coordination learning activities, the learner will gain
the knowledge and skills needed to:
• Make clinical decisions through application of the nursing process and critical thinking.
• Set priorities based on the identified needs of one patient.
• Incorporate effectiveness and efficiency into the delivery of care.
• Plan for cost-effective care to one individual.
• Effectively manage time while providing care to one patient.
• Incorporate ongoing evaluation of the patient’s response to care, adapting plan as
priorities change.

Value of Clinical Decisions
The nurse uses clinical decision making to keep THE HEALTH CARE TEAM

focused on ACTIONS THAT MOVE THE CLIENT TOWARD HEALTH.

of action, thus increasing the chance that clients will have optimal health outcomes.

If the nursing plan of care is based on inaccurate data and poor clinical decisions about a client,

When admitting a client to an acute care facility (hospital), the nurse addresses safe keeping of
the client’s belongings. Place the actions in the order in which the nurse completes them, starting
with the first.




• Ask the client what belongings they brought with them.
• List the belongings in the electronic health record (EHR), with as much detail as possible
for accuracy.
• Advise the client that all items of value should be sent home for safe keeping.
• Note the disposition of items on the inventory within the EHR.
• Document that the client was informed of valuables policy and elected to dispose of items
as noted on the Valuables Inventory.

Clinical Care Coordination
Many of the skills required by the nurse to coordinate and deliver care to patients are the same as
those used by the student nurse to balance their learning with the rest of their life: making
decisions, setting priorities, staying organized, managing resources, making the most of the time
available, and evaluating how things are going to see if changes need to be made.
