DIABETES

- Diabetes Mellitus Tipo 1 → dependencia absoluta de insulina, Tx: reemplazo de insulina
- Diabetes Mellitus Tipo 2 → respuesta disminuida a insulina, Tx: agentes orales (1ra línea:
metformina), inyectables no-insulina, reemplazo de insulina.


• INSULINA: ↓ liberación de glucagón desde las células α. ↑ retención de Na+. ↑ síntesis y
almacenamiento de glucógeno y triglicéridoa. ↑ síntesis de proteínas en los músculos.


• REEMPLAZO DE INSULINA: unión a receptor de insulina (actividad de tirosina quinasa). Insulina
regular (de acción corta) para cetoacidosis, hiperkalemia e hiperglicemia por estrés.

Efectos adversos: Hypoglicemia, lipodistrofia, reacciones alérgicas (raro), aumento de peso.


Acción rápida Lispo, Aspart, Glulisina Insulina modificada Peak: 1-2hr Duración: 3-4hr

Acción corta Insulina regular Insulina no modificada Peak: 2-3hr Duración: 6-8hr

Acción intermedia NPH modificada, protamina Peak: 4-10hr Duración: 12-18hr

Acción larga Glargina, detemir Insulina modificada No tiene Duración: 20-24hr



BIGUANIDAS ↑sensibilidad
a insulina
Metformina

↑ sensibilidad a insulina. ↓ gluconeogénesis hepática. ↑ glicólisis o captación
Mecanismo
periférica de glucosa. Se asocia con pérdida de peso → pérdida del apetito.

Efectos Distrés gastrointestinal, acidosis láctica (sobre todo en insuficiencia renal),
secundarios hipoglicemia, deficiencia de vitamina B12

TIAZOLIDINEDIONAS ↑sensibilidad

Rosiglitazona — Pioglitazona

Se une a PPAR- γ, ↓ gluconeogénesis y triglicéridos, ↑ número de receptores para
insulina. No tiene efecto directo sobre la secreción de insulina, sino que amuenta la

sensibilidad de tejidos a la insulina. Aumentan HDL, Pioglitazona ↓ Tg,
Rosiglitazona ↑ Tg y LDL.
Efectos
Aumento de peso, edema, fallo cardiaco, Osteopenia: fracturas.
