SUBJECTIVE
CC: “I have purulent discharge for penis.”
HPI: A 25 year old Hispanic male presents to the office with complain of purulent
discharge. The patient says that the symptoms are also accompanied with burning
sensation on urination. The patient says that the symptoms started 3 days ago after
engaging in unprotected sex with the girl he made in the club. The patient pain that he
feels when urinating as 5 in a scale of 0 to 10. The patient says that his symptoms seems
to continue being severe. The patient says the penile discharge is continuous and he only
feels burning sensation when passing urine. The patient says that he was diagnosed and
treated for chlamydia about 8 months ago and was treated with antichlamydial antibiotics.
The patient also states that he finds it difficult to maintain one sexual partner. The patient
states that he has never been diagnosed with any chronic ailment. The patient denies
changes in energy, weight loss, fever, night sweats and chills. The patient says that he has
not taken any form of medication to relieve his symptoms and he decided to come for
treatment after thinking his condition might be serious.
Medications:
Azithromycin 1 g orally as a single dose.
