CC: “Itchy vaginal with foul-smelling odor and whitish vaginal discharge.”
HPI: A female aged 32 comes to the office and complains of itchy vaginal with foul-
smelling odor and whitish vaginal discharge. The patient adds that the she has also been
experiencing painful urination and states that the signs and symptoms of her condition
started 3 days ago. The patient states that that her pain is mild and can be classified as
4/10. The patient reports that the vaginal smell and discharge is continuous but the itchy
feeling comes and goes. The patient reports feeling uncomfortable since the itchy feeling
makes her want to itch herself in public. The patient also reports being sexually active
and denies recent diagnosis of any health condition. The patient also denies past
diagnosis of chronic ailment. The patient denies taking any medication to relieve the
symptoms.
