In NR 500, you developed a PICOT question.
Select a nursing theory that you find useful in
nursing practice. Discuss each of the following
selected elements of the PICOT format by
applying your selected nursing theory to the
element. Be sure to include a scholarly reference.
• Population/ Patient Problem: Consider how your selected nursing theory may guide you in selecting the
population and/or patient problem
• Intervention: Does your selected nursing theory identify a possible intervention? If not, how is your
selected intervention consist with your selected nursing theory?
• Outcome: How would the outcome of your PICOT study contribute to your selected nursing theory? Would
it add new information? Would it confirm some part of the theory?

Nursing theory applied to research
Dr. Rasmussen and class,
I have to admit, through the work done for this theory class, I’ve become enamored with
Betty Neuman’s systems model. I find it has a broad range of application and can be related to
many concerns within the nursing profession. It can be easily applied to the elements of the
PICOT format. My PICOT question in NR 500 was: In patients undergoing surgery, is forced air
warming more effective than resistive cutaneous warming in preventing perioperative
hypothermia? This question was a specific practice intervention question. My application of
theory to practice topic for this class, while not a formal PICOT question, also relates. My topic
,related to using educational workshops to build resistance and prevent burnout and turnover in
nursing.
The “P” component, population/patient problem is easily identified. The Neuman systems
model considers the client as a system, which can be patients, patient problems, communities, as
well as broad reaching universal issues (Neuman & Fawcett, 2011). This definition would allow
for a wide selection and application. In my PICOT question, the “P” component was patients
undergoing surgery. In my application topic it was nurses in general.
For the intervention component, the Neuman theory identifies nursing interventions on
three levels for assisting a system to effectively deal with stressors (Turner & Kaylor, 2015). The
first level is primary, which involves health promotion and prevention interventions. The
secondary level involves interventions that strengthen a systems lines of defense mitigating stress
responses. Tertiary interventions maintain health and offer further protection. In my PICOT
question, specific warming methods are both primary and secondary interventions. In my
practice application, educational workshops could be considered interventions on all three levels.
The Neuman model can assist as a framework for evidence based interventions to practice and
nursing problems (Turner & Kaylor, 2015).
The ideal outcome of the Neuman model is optimal health or stability of a system
(Turner & Kaylor, 2015). Neither the outcome of my PICOT, prevention of hypothermia, nor
does the outcome of my practice problem strategy, strengthened resilience, contribute to the
theory or provide new information. However, if successful, they would confirm the outcome of
establishing and maintaining stability or health through interventions targeted at the lines of
defense and resistance as proposed by the theory.

, Catherine thank you for a well-designed research methodology. In one of your paragraph,
you stated that I’ve become enamored with Betty Neuman’s Systems Model. I agree I am also
bewildered by the Betty Neuman’s System Model, in particular with the compound rings. I
believe the reason why this theory is great is that it is a nursing model rather than a real theory. It
concentrates on the response of the patient body to environmental stressors and theorizes that if
stress and tension cannot be barred, then the nurse should simply protect the patient's primary
structure to enable the individual to achieve the maximum level of wellness (Neuman & Fawcett,
2012). It does this by using several planes of nursing preventive strategies to achieve, preserve
and sustain the patient highest level of wellbeing (Neuman & Fawcett, 2012). These several
planes can thus be applied in designing various PICOTS model to create multiple sustainable
models of patient preventive care strategies. Once again thanks for your posting

Thank you for posing such a thought provoking question. My PICO(T) question
regarding forced air warming was more effective in comparison to cutaneous warming was
certainly a yes or no question. When considering how to rephrase this research question, I tried
, to change it into a format that would be broader in response. One possible rephrasing option that
I came up with is: In patients undergoing surgery, how effective is cutaneous warming in
preventing perioperative hypothermia in comparison to forced air warming?
When considering the practice problem I discussed regarding utilizing workshops to
prevent burnout and turnover in nurses, I came up with a question in PICO(T) format addressing
this application. How effective are formal resilience skill building workshops in reducing the risk
for burnout in experienced nurses? Other options could include comparing the effectiveness of
different skills or workshops.
It is certainly easier to ask a yes or no question. These questions rephrased may offer
more insight during the research.

Applying Orem’s Theory & PICOT in designing reach based strategy to prevent
Hyperosmolar Hyperglycemic State (HHS) in our LTAC Diabetic patients
I am an intensive and rehabilitative nurse with background experience in LTAC Nursing
Rehabilitation. I have particular experience with patients who because of poor diabetes control
have developed all types of complication including acute complication such as Hyperosmolar
Hyperglycemic State (HHS) and Diabetic ketoacidosis (DKA). They also develop chronic
complications such as CAD, renal failure, HTN, DM foot, and PVD. Patients continue to run
high sugar even after admission to our LTAC. The main reason for poor diabetes control is
because of inadequate information on diabetes education to help diabetic patients achieve self-
care needs.
Orem’s self-care deficit theory is a perfect method to use in fixing the knowledge deficit
among our LTAC patients. Orem theory has three separate interrelated sub-theories the theory of
self-care, the self-care deficit theory, and the theory of nursing systems. As per Orem's theory,
the supposition is that when patients are unable to meet their self-care requirement, a self-care
deficit occurs and nursing care is needed to help correct to those self-care deficits. Orem’s self-
care theory provides nurses with a rock solid foundation for helping patient and giving them the
chance to demonstrate individuality and exercising control in caring for themselves. Orem Self-
care theory assumes that people should be self-reliant and responsible for their care and the care
of others in their family (Philbrick, 2012). It emphasizes that patient’s knowledge of potential
health problems is necessary for promoting self-care behaviors. Patient health education
