I have chosen to discuss the acquisition of a 25-physician multispecialty practice discussed in C432, Applications in Healthcare Management and Strategy. A2. Identify one of the following professional lenses: payer, manager, technology professional or clinical professional. I have chosen to use the Manager lense in this scenario; the 25-physician multispecialty acquisition. Specifically, the Change manager responsible for implementation and monitoring of the new team-based model of care for the organization. A3. Summarize the opportunity selected in part A1 from the perspective of the professional lens selected in part A2. With every acquisition, there are some growing pains. When the team-based model of care was introduced, all work groups have performed well in providing quality care. As discussed during the implementation of this new model, new positions may be added to assist staff in keeping care focused on the patient, from admission to discharge. As this process is evaluated constantly, it appears that we need to add a Care Coordinator position. This individual is responsible for coordinating the transition of care when applicable. This means, for example, a cardiac patient that comes into the Emergency Room will be assigned a Care Coordinator from the Cardiac Department. The Care Coordinator will follow this patient from the Emergency Room to the Cardiology Department and Cardiac Therapy; being the main contact for the patient. This limits the myriad of numbers the patient must call to find out what they are to do next in the treatment regimen. This will reinforce to the patient that their well-being is at the forefront of the organization. A4a. Describe the process as it is being performed today. When patients come into the hospital for treatment, regardless of department, they receive quality care from both clinical staff (Nurses, Medical Assistants, Physicians, etc.) and non-clinical staff (registration, admissions, etc.). Transfers to different departments (Emergency Room to Cardiology) are smooth with electronic medical record access available to all departments. When the patient is discharged, discharge instructions are discussed, explained and given to the patient with contact information to the clinic or physician that is next to follow up with the patient. On the instructions is a link to rate their patient experience regarding their care from beginning to end.
