-Polypharmacy is an area of concern for elderly because of several reasons. Elderly
people are at a greater risk for adverse drug reactions (ADRs) because of the metabolic
changes and reduced drug clearance associated with ageing; this risk is furthermore
exacerbated by increasing the number of drugs used.
● What is the health concern for older adults with
Polypharmacy?

● What is medication reconciliation and when should it
be done? How does medication reconciliation relate to
Polypharmacy?
-Medication reconciliation is the process of creating the most accurate list possible of all
medications a patient is taking — including drug name, dosage, frequency, and route —
and comparing that list against the physician’s admission, transfer, and/or discharge
orders, with the goal of providing correct medications to the patient at all transition
points within the hospital.


● Describe the Beers Criteria. How can nurses use the
Beers Criteria to keep older adult clients safe?
-The Beers Criteria were first developed in 1991 by Mark H. Beers, MD, to decrease
inappropriate prescribing and ADEs and, in particular, to identify medications or
medication classes that should be avoided in older adults in nursing homes.
Nurses play a key role in confirming that patients are prescribed the appropriate
medications. They do so not just by checking to make sure a drug's indication matches
the patient's diagnosis, but also by considering the patient's age, height and weight,
comorbidities, and other medications (prescription and over the counter) prior to
administering the drug. Nurses also assess the appropriateness of a medication when
