I. C/C: U.P. is a 3YO/F whose mom is complaining that the child has a red rash on her arms and
cheeks that seems to itch. This has been going on for months and she has tried Vaseline and
topical antibiotic ointments without success. She also says the child has a constant runny nose. - -
Case study 1
-Atopic dermatitis (Primary Diagnosis) - -Pruritic lesions in infant, erosions with serous
exudates, papules, or plaques, symmetrical over the face, hand, and trunk.
Atopic Dermatitis (Atopic Eczema) The description of the rash matches the description of atopic
dermatitis. In childhood (ages 2-12), atopic dermatitis is often manifested as pruritic papules, and
plaques with excoriation are common, both of which the patient has.
dermatologic finding is associated with asthma, allergic rhinitis, and eczema
-Fifths Disease not primary - -The child presents with a facial rash or a "slapped cheek "
appearance, but no fever the rash is spreading to the trunk and limbs.
-Atopic Dermatitis Pathophysiology - -It is IgE mediated and occurs as an allergic response
when an allergen comes in contact with the skin.
-Langerhans Cells - -Epidermal macrophages
-C/C: Mom brought to the clinic a 4 YOM child who was sent home from school with a rash. No
others in the family are ill, but this child has been "a bit tired and not his usual busy self" for 1
week.
PMH: Negative
ROS: Negative except for rash.
PE: Skin: circular pink patches of scaly papules on trunk with one larger 3x4cm patch on upper
scapular area. Pruritus noted with excoriation.
VS: WNL - -Case study 2
-pityriasis rosea (Primary Diagnosis) - -Presents with a herald patch, Christmas-tree pattern.
usually a virus, can look like fungus or dermatitis.
cause unknown, self limiting illness lasting 4 weeks to 8 weeks and asymptomatic. Pt c/o oval
lesions with fine scales that follow skin lines(leavage lines) of the trunk or a "christmas tree"
