The chosen community health nursing diagnosis statement pertains to the
promotion of health and well-being in the disabled population, emphasizing access to
disability-related services and devices. Increased risk of inadequate healthcare and
resources among the disabled population related to a lack of education and services as
demonstrated by the data shown in the community reports, the data that is mentioned
later in this paper, the lack of understanding in the general public, the lack of
knowledge in the entire community about the resources in that same community, and
the need for increased resources and funding in the area. The concern is that the
disabled population in the community does not receive enough support or services to
have the best quality of life. This is because of the findings from a Windshield
Assessment. Therefore, there are health inequities and disparities in all realms of the
disabled. This is shown in the lack of technology provided to or for the disabled, a lack
of healthcare for the able-bodied and the homebound, a lack of food or nutrients for the
disabled due to lack of funding or ability to get to the store, and the lack of education
provided to the public so there may be a change that helps the disabled. Overall, there
is a lack of healthcare, a lack of technology, a lack of assistance, and a lack of
understanding in the disabled community. Most of the disparities for this population are
due to a difference in socioeconomic status, geographic location, a difference in
education, and age, which all affect their disparities related to the disability aspect.
The Healthy People Campaign is one that has been attempting to bridge the gap in
disparities for this particular population (HP 2020, 2022). Because these disparities are
barriers to care, the community then needs to come up with resources and technology
to assist this population. These are all issues that are being addressed by one
community entity or another, however, they still need vast amounts of work.
Some of the available community resources come in different forms. One main one
providing many sub-resources is the Community Center. This provides classes, meals, a
