Introduce yourself to the client.
Perform hand hygiene and put on appropriate PPE,
if indicated.
Verify client identification.
Determine whether the client has allergies.

Head Circumference
STEP S U EVALUATOR’S COMMENTS
Lay infant supine on padded examination table.
Using non-stretchable tape measure, measure
widest part of infant’s head.
Place tape measure across forehead above the
eyebrows and ears and wrap tape measure
around infant’s occipital prominence.
Hold tape measure taut and note measurement.
Remove tape measure.
