Assessor will call DM or cellphone 2. Read off own packet (do not crowd the child but periodically check to for response sets) 3. Do not fill out out questionnaires with child in CCF waiting room, or when they're distracted 4. Child might feel more comfortable -do not give advice -#9 on CDI or #8 on CDI-2!! important to let assessor know immediately before family leaves 5. Validity is important - tell them to tell you what they need to do before each measure -how would you feel or think if you were ever in that situation? -same format, monotone voice 6. if the child says they don't have contact with a parent or if the parent is deceased -confirm with assessor after child completes all other measures, if assessor agrees mark X on the page 7. Feel uncomfortable with the child or the child is not responding stop what you are doing and contact assessor for further guidance 8. check questionnaire packets for completion Trouble shooting issues with Assisting children with questionnaires - 1. if the child days she/he doesn't need help, tell them that everyone gets the same type of help and it makes things easier/faster for them 2. play for 5 minutes if needed 3. taking small breaks between measures
