Discuss how to initiate a quality improvement project for this clinical problem. Lauren firstly needs to explain and describe the project initiative project in a short and concise manner remembering all the important parts. For example, patients are not receiving full therapy treatments on a timely basis, and this has caused a decline of patient care and quality of service. As a result a quality initiative plan is being established to find ways to aide patients with full therapy treatments (i.e. occupational, physical, and speech therapies) as they were intended. Lauren then needs to carefully choose and put together a quality initiative team. CEO is usually the sponsor, the head nurse can be the QI team lead, Lauren can keep herself as the QI team expert, other professionals like transport experts, the therapy team such as the lead of occupational therapy, physical therapy, speech therapy should be included. A person with an outside perspective should also be invited, maybe the head of a department that handles scheduling and can understand the scope of the situation. The project manager for this assignment should also be Lauren, since its her project. Identify three areas of focus for quality improvement in the scenario. Three areas of focus to improve the quality from the scenario can be.1. The Patients to get their full prescribed therapy sessions 2. To make sure that the patients get their nursing needs in a timely manner and to get the coordination of the nurses aides to get to the therapy sessions on time each day. 3. Coordinated scheduling and communication between the nursing staff and the therapists. a. Describe the types of data gathered in part A1 (e.g., quantitative data). The types of quantitative data needs for part A1 would be the timings and lengths of time for every patient for how long each of their therapy session takes, what time they usually wake up in the morning, how long it will take them each morning to get ready and how long it will take them to get their nursing needs. All these times and lengths of time are very important to implement the new quality improvements. Additionally Lauren will need the schedules of the nursing staff, the nursing aides, the therapists. Lauren will need to know which patients the aides and nurses are assigned to, Lauren will need to know the therapy assigned to each patient and to which therapists and the lengths and times and which days. Lauren will need a through picture of the full schedule of the patients that need the hour long therapy and the full schedules of all that work with them, including the nurses, the aides, the transporters, and the therapists. b. Explain your process for gathering the data. Lauren will have to get the help of the nursing staff, the nursing aides, the therapists, and together she will have to gather the quantitative data needed to help improve the quality goals she and her team of QI professionals set out to accomplish. Additionally, Lauren will need to check all data and scheduling for all of the nurses, nurse’s aides, and therapists, she will have to sit down with her QI team and gather relevant information from them about the quality improvement initiatives. Lauren would have to gather data from previous times when getting a full hour therapy was successful and ask the staff on what has changed. This would mean several meetings, interviews, and real-time observations of these patients and the staff that help them day to day, Lauren will later have to organize all this information into a graph or chart for her reference. 2.
