5 Requirements of a Treatment Plan: - ANSWER-1. Appropriately address the patient's CC 2. Include treatment modifications for any medical conditions that may affect the delivery of dental care. This includes that if medications are required you must plan the drug, dose, and directions for use. 3. Recommend any necessary additional diagnostic tests or referrals to specialists when indicated including a diagnosis and proposed treatment. 4. Be comprehensive and appropriately sequenced to address all of the patient's needs. 5. Be succinct, organized, and readily interpreted. 5 Grading Criteria for Tx Plan: - ANSWER-1. Treatment modifications: all modifications are appropriately noted (medication, referral, etc.). Specific medication dosage is required. 2. Is the tx plan inclusive?: All items that must be addressed and the patient's CC is correctly addressed. 3. Does the tx plan exhibit overtreatment?: Only those item(s) that must be addressed are addressed. 4. Is the tx sequence appropriate?: The sequence is optimal. 5. Is the tx plan concise, well-organized, and easily interpreted? ***Each plan is scored out of 5 total points and an average scrore of 3 is required for passing ***Critical errors result in failure regardless of the score and are considered to be anything that is likely to cause life-threatening harm or severe morbidity resulting in hospitalization.
